About The Position

This role is for a Human Services Professional in a residential treatment setting, focusing on Clinical Case Coordination. The position involves ensuring the educational, medical, emotional, and social needs of residents are met. The Human Services Professional will monitor resident needs, arrange for appropriate services, provide case work services as per service plans, and monitor educational/vocational needs. This role also involves planning, coordinating, and documenting services for residents and their families, modeling life skills, and providing feedback. Overall responsibilities include case management, community support, and administrative duties. The professional will provide full clinical case management services, coordinate with internal and external contacts for community resources, and work closely with Unit Supervisors to ensure coordinated care. Responsibilities also include coordinating discharge planning and meeting with family members, regulatory agencies, and case workers. On-call duties are also part of the role. Requirements

  • Bachelors in psychology, social work or other related field.
  • Minimum two years previous experience in behavioral healthcare setting, including internships preferred.
  • Excellent verbal and written communication skills.
  • Ability to handle multiple tasks and meet deadlines with minimum supervision.
  • Self-motivated individual.
  • General knowledge of clinical best practices for working with adolescent populations and their families/resources.
  • General knowledge of mental health, funding and related services within Georgia and other states served.
  • General knowledge of standard office computer applications and ability to learn new programs.
  • Valid Driver's License.
  • Must be at least 21 years of age.

Responsibilities

  • Ensure that the educational, medical, emotional, and social needs of the resident are met.
  • Monitor the resident’s needs and ensuring that appropriate services are being provided and arranged for in order to meet those needs.
  • Provide case work services as provided in the resident's service plans.
  • Monitor of the resident's educational and/or vocational needs.
  • Plan, provide, arrange, coordinate, and document service to residents and their families.
  • Model life skills and provide activities for youth to practice life skills and provide appropriate feedback.
  • Assume overall responsibilities for Case management; assumes role of community support as well as administrative responsibilities.
  • Provide full clinical case management services to clients.
  • Coordinate and cooperate with both internal and external contacts/sources to ensure the clients have access to community resources as required.
  • Work closely with the Unit Supervisors to ensure care is coordinated and provided.
  • Coordinate discharge planning.
  • Meet with family member, regulatory agencies and case workers as needed to facilitate discharge plans.
  • Perform on-call duties as scheduled.
