About The Position

Under the supervision of the HUD Director, the HUD Service & Resident Benefit Manager links the people supported to supportive, benefit, and/or medical services provided by private and/or public agencies. The HUD Service & Resident Benefit Manager may organize a functional assessment of the clients’ abilities and participate in developing Individual Habilitation Plans. This role also always serves as an advocate for the Agency’s clients.

Requirements

  • Bachelor’s degree in human service field from an accredited college or university, or related field of intellectual disabilities, or other appropriate work experience.
  • Position requires a valid driver’s license, proof of Agency-approved insurance carrier and coverage, and the provision of a motor vehicle report acceptable to the Agency’s insurance broker.
  • Must be willing to travel as necessary.
  • Mandatory, regularly scheduled training and continuing education in the aging process, elder services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs, legal liability issues relating to providing service coordination, drug and alcohol use and abuse by the elderly, and mental health issues is required, but is not a prerequisite for hiring.
  • Able to exercise leadership, provide directions and guidance, and present a positive image at internal and external meetings, trainings, activities, and events.
  • Willingness to foster agency, department, and program-wide cooperation and teamwork through positive/constructive communication techniques.
  • Able to facilitate a positive, integrated, cohesive, and supportive person-centered approach and attitude among all program personnel.
  • Strong organizational skills and ability to manage multiple tasks.
  • Demonstrated verbal and written communication skills.
  • Proficient in computer skills and mastery of Microsoft Office applications.

Responsibilities

  • Coordinate with the healthcare coordinator to arrange medical and related appointments for all persons supported in the community living program.
  • Ensure that residential, medical, behavioral, and other clinical portions of the healthcare coordinators are completed for services provided or contracted by Ada S. McKinley.
  • Provide direct supervision, coaching, feedback, and evaluation to healthcare coordinators with the completion of his/her job duties.
  • Provide strategic oversight and management of all public benefits coordination for individuals, including but not limited to Social Security, LINK (SNAP), Medicaid, and Medicare programs for both the Community Living Program and Community Day Service Program.
  • Supervise and direct the activities of the Healthcare Coordinators, ensuring adherence to organizational policies, Professional standards, and applicable regulatory requirements.
  • Monitor and ensure the completion of all benefit-related applications, renewals, and documentation in accordance with established timelines and procedures.
  • Coordinate monthly medical appointment review with the community living team.
  • Facilitate effective communication and coordination with government agencies and service providers to support timely and accurate access to benefits and healthcare services.
  • Develops systems to track and maintain records on the impact of programs for residents.
  • Participate in the surveys and inspections conducted by applicable regulatory and lending bodies.
  • Attends trainings and seminars out of the state at least twice per year, per the U.S. Department of Housing and Urban Development’s (HUD) requirements; such other tasks, responsibilities, and training which are required or may be created by the U.S. Department of Housing and Urban Development (HUD) in connection with HUD’s funding, implementation, and monitoring of the service coordinator program.
  • Travel as required for seminars and other agency events.
  • Performs other tasks and duties related to the position as required and assigned.
