About The Position

Christie Ossington Neighbourhood Centre (CONC) is a multi-service community organization supporting individuals and families through shelter, housing, food security, and community-based programs across Toronto. The Housing Worker provides direct support to individuals accessing CONC’s Hostel and Drop-In services, helping them find and maintain safe, sustainable housing. As a key member of the Breaking Barriers team, this role involves collaborative case management and outreach to landlords and community partners. You will work closely with the Life Management, Health & Wellness, Income Security, and Drop-In teams to deliver integrated, client-centered housing solutions. This is a unionized position reporting to the Supervisor of Shelter Programs.

Requirements

  • Diploma or degree in social work, community services, or a related field.
  • Minimum 2 years’ experience providing housing, advocacy, and/or case management support.
  • Strong knowledge of Toronto’s housing system, landlord-tenant law, and community resources.
  • Experience with eviction prevention, housing outreach, and mediation.
  • Ability to engage individuals with complex needs, including mental health, addictions, and criminal justice involvement.
  • Effective interpersonal, written, and verbal communication skills.
  • Demonstrated ability to work collaboratively in an interdisciplinary team.
  • Proficiency in MS Office and client documentation systems.
  • Strong organizational skills and professional boundaries.

Nice To Haves

  • Conflict resolution and crisis intervention training preferred.

Responsibilities

  • Assess client needs and develop personalized short- and long-term plans of care.
  • Deliver one-on-one housing support, including landlord outreach and securing permanent housing.
  • Provide housing-focused case management to address barriers and promote housing stability.
  • Conduct mediation and eviction prevention interventions as required.
  • Coordinate supports across internal teams (Health & Wellness, Life Management, Income Security).
  • Build relationships with landlords, property managers, and housing providers.
  • Develop and maintain a landlord inventory and community housing partnerships.
  • Promote housing opportunities within the shelter and drop-in settings.
  • Offer system navigation and advocacy support related to housing, social assistance, and tenancy rights.
  • Support clients in developing self-advocacy and independent living skills.
  • Train and support new staff, students, and volunteers as assigned.
  • Maintain complete and timely documentation, including physical files and electronic housing reports.
  • Participate in relevant meetings, community tenancy forums, and internal committees.
  • Assist with front desk coverage and floor support as needed during staff shortages.
  • Adhere to all CONC policies including confidentiality, code of conduct, and hostel standards.
  • Attend required staff training and development opportunities.
