Under the supervision of the Director of Housing and Outreach, the Housing Support Case Manager provides comprehensive case management services to single adults and or families living within Eva’s Shelters, transitional housing programs or Passaic County. Housing Support Case Manager assists Medicaid-eligible individuals with obtaining and maintaining safe, stable, and affordable housing while promoting independence, improving health outcomes, and reducing unnecessary hospitalizations, emergency room visits, and institutional placements. All qualifying individuals will be considered chronically homeless or facing housing insecurity with active Medicaid. Case managers will assist individuals with presumptive eligibility screenings, maintaining active benefits and MCO Authorizations. Case managers will be tasked with providing person-centered, trauma-informed services that address social determinants of health through comprehensive care coordination, housing stabilization, advocacy, and collaboration with healthcare providers and community partners. Case managers will draft and monitor service plans for each client and or family. This role is specifically attached to the Medicaid housing support initiate and funded through Medicaid billing. This role will require field work in combination with work across multiple programs within Eva’s village.
