About The Position

Wraparound Maryland, Inc. is seeking a dedicated, resourceful, and empathetic Housing Stabilization Specialist to provide direct care and non-clinical case management for the Pathways Home program. This community-based initiative serves approximately 25 low-income households and 80 unduplicated individuals annually in Worcester County, specifically targeting individuals, families, and Transition-Aged Youth (TAY, ages 18–30) facing housing instability, system transitions, and economic hardship. The Specialist works directly on the front lines to help participants navigate the transition from homelessness, incarceration, foster care, or institutional settings into stable, long-term housing and self-sufficiency.

Requirements

  • Direct care and non-clinical case management experience.
  • Experience working with low-income households, individuals facing housing instability, system transitions, and economic hardship.
  • Experience working with Transition-Aged Youth (TAY, ages 18–30).
  • Ability to help participants navigate transitions from homelessness, incarceration, foster care, or institutional settings.
  • Proficiency in coordinating housing relief, including managing and distributing resources like rental assistance, security deposits, and utility support.
  • Skills in conducting regular check-ins and assessments to identify barriers to stability.
  • Ability to assist participants in building long-term self-sufficiency goals.
  • Experience navigating complex adult systems.
  • Capability to link participants to critical community resources (behavioral health, employment, healthcare).
  • Experience assisting individuals in obtaining vital records (birth certificates, state IDs, Social Security documentation).
  • Experience overseeing and facilitating drop-in supportive services.
  • Ability to provide access to basic needs like showers, hygiene supplies, and refreshments.
  • Understanding of tailoring care approaches to specific populations (homeless, justice-involved, transitioning from foster care/treatment).
  • Must be able to work in Worcester County, focusing on high-need areas including Zip Code 21811.

Responsibilities

  • Coordinate immediate housing relief by managing and distributing program resources, including rental assistance, security deposits, and utility support to prevent displacement and reduce homelessness.
  • Conduct regular check-ins and assessments to help participants identify barriers to stability. Assist in building long-term self-sufficiency goals and navigating complex adult systems.
  • Link participants to critical community resources, including behavioral health services, employment opportunities, and local healthcare networks.
  • Directly assist participants in removing barriers to employment and housing by helping them obtain vital records, including birth certificates, state identifications, and Social Security documentation.
  • Oversee and facilitate low-barrier drop-in supportive services. Provide participants with access to showers, hygiene supplies, and light refreshments to build trust, maintain dignity, and encourage continuous program participation.
  • Tailor care approaches to meet the distinct needs of priority populations: Individuals and families experiencing or at imminent risk of homelessness; Justice-involved individuals returning to the community post-incarceration; Adults and TAY (ages 18–30) transitioning out of foster care, residential treatment, or behavioral health facilities.
