About The Position

OCES is growing and expanding its services to the south shore area, including Quincy, Braintree, and Weymouth. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. The organization's mission is to promote healthy, safe living, allowing consumers to stay active and engaged in their communities. The Housing Specialist will work within the Housing team to ensure services and support to residents on-site in South Shore area locations.

Requirements

• At least two years of experience in the human services field, preferably with the elder population
• Excellent written and verbal skills
• Excellent computer skills
• Proficient with Microsoft Office Suite or related software

• Adaptability to be able to change as the needs of consumers and/or programs change
• Bachelor's degree in social work or related field preferred
• Bilingual a plus

Responsibilities

• Review the needs of residents to link the community programs and services
• Serve as a multi-disciplinary assessment team leader and meet with the team to assure service coordination
• Facilitate care conferences with residents, family, and other participants
• Plan and coordinate social, educational, and recreational programs for residents
• Prepare and distribute a newsletter to keep residents informed of planned activities
