About The Position
Trillium Health Resources is hiring a Housing Specialist II - TCL to support individuals in securing and maintaining housing. The role coordinates housing services and develops housing plans, working with providers, landlords, and community partners to align resources, address barriers, and support successful housing placement and retention. The position is remote within Trillium’s Mid State Region, which includes Anson, Guilford, Montgomery, Randolph, and Richmond counties, and requires travel within the catchment area as needed.
Requirements
- Bachelor’s degree.
- Two years of experience working with individuals and the housing systems serving people with SMI/SPMI to obtain and maintain Permanent Supportive Housing (PSH).
- Valid driver’s license.
- Must reside in Trillium’s Mid State Region: Anson, Guilford, Montgomery, Randolph, or Richmond County.
- Must be able to travel within the catchment area as required.
- Must be legally authorized to work in the United States without requiring employment sponsorship now or in the future.
Responsibilities
- Coordinate housing services to support placement and long-term housing stability.
- Develop and implement housing plans based on individual needs and available resources.
- Work with landlords, providers, and community partners to secure and maintain housing arrangements.
- Monitor housing stability and address issues that may affect placement or retention.
- Facilitate access to housing resources, supports, and related services.
- Provide guidance on housing options, requirements, and ongoing responsibilities.
- Maintain documentation of housing planning and coordination activities.
