Requirements
- High school diploma or GED
- 3+ years of experience in services for older adults, people with disabilities, or social services in a healthcare setting
Nice To Haves
- Bachelor's degree
- 5+ years of experience in the above
Responsibilities
- Connect residents and home-based non-residents with the right supportive services, and help coordinate transitions to higher levels of care when needed
- Check in regularly with the most vulnerable residents, including follow-up care after a hospital stay or therapy
- Build and maintain a resource guide of local services, and prepare monthly and annual reports (including HUD reporting)
- Create newsletters, website updates, and other communications, and serve as the point of contact for resident groups
- Plan events, classes, and programs (social, spiritual, and vocational) that help residents stay engaged and connected
- Work with management to help residents stay in compliance with their lease and HUD requirements
