About The Position

Reporting to Director of HIV and STD’s, the Housing Project Coordinator for the PRIME Program will be responsible for the maintenance, and growth of the housing services for people living with HIV. This position will oversee day-to-day operations, including provision of case-management & outreach, health and psychosocial education, linkage to care for patients experiencing homelessness, patient advocacy, liaison with clinical, operations staff as well as with partner agencies and funders such as APLA.

Requirements

  • Minimum 2 years of professional experience managing social service and/or public health programs in community.
  • Minimum 2 years of professional experience with advocacy and policy change initiatives.
  • Experience working with people experiencing homelessness, transgender and gender nonconforming people and familiarity with the target population.
  • Knowledge of homeless community resources and social centers in Southern California.
  • Experience working in underserved communities such as PLWHA (Person living with HIV/AIDS), undocumented individuals, unaccompanied minors, LGBT populations, communities of color, youth/adolescents, and those experiencing homelessness, substance abuse, and/or mental illness.

Nice To Haves

  • Preferable Bachelor’s degree in social work, psychology, public health, or a related field or equivalent experience required.
  • Bilingual English/Spanish preferred (read, write, speak)

Responsibilities

  • Oversee day-to-day operations and support service provisions of housing with APLA and PRIME
  • Ensure grant/contract deliverables and outcomes are monitored and met.
  • Serve as point of contact for patients, staff, partners, and funders.
  • Coordinate programmatic activities, including policy and advocacy.
  • Create necessary infrastructure to advance program, including coordination with St. John’s providers and staff, as well as leadership and staff of partner agencies.
  • Maintain up-to-date knowledge and database of community resources and referral options for homeless patients.
  • Coordinate, and monitor program evaluation activities.
  • Collect the necessary data to report on achievement and evaluation of grant goals and objectives.
  • Submit monthly reports on grant-related activities.
  • Create presentations, trainings, and technical assistance tools for St. John’s providers and staff.
  • Attend meetings, trainings, and other work-related events as needed.
  • Administrative duties as assigned by supervisor.
  • Other duties may be assigned or may be modified as business needs dictate.
