Housing Navigator - Pathways
About The Position
Catholic Charities of the Archdiocese of Newark is seeking a Full-Time Housing Navigator for their Pathways to Housing program located in Newark, NJ. This role involves coordinating services and facilitating access to housing and supportive services for clients. Referrals will be made to appropriate community-based providers based on individual needs. The staff will assist clients in developing and managing individualized Housing and Service plans, including direct coaching, counseling, information, referral, linkage, care coordination, and direct services. The position is also responsible for follow-up on referrals and services to ensure residents access care and to address needs as necessary.
Requirements
- Bachelor’s Degree in human services or related field
- 1-2 years of related experience in human services field and/or housing.
Responsibilities
- Coordinate services and facilitate access to housing and supportive services.
- Refer clients to appropriate community-based providers based on their individual needs.
- Provide clients with development and management of individualized Housing and Service plans.
- Offer direct coaching and counseling services.
- Provide information, referral, and linkage.
- Coordinate care.
- Provide direct services to individuals.
