About The Position

Copa Health is seeking a motivated individual to Perform outreach services within the valley and engage with vulnerable individuals experiencing homelessness with a Serious Mental Illness (SMI) designation or those at risk of health concerns. The role focuses on connecting individuals with health and wellness resources, as well as housing services. This position involves conducting outreach in parks, faith-based communities, libraries, nonprofit organizations, heat relief stations, freeway underpasses, and similar locations.

Requirements

  • High school diploma or GED required.
  • At least one year of experience as a Navigator or working with people experiencing homelessness.
  • Knowledge of community resources in Arizona.
  • Fingerprint clearance card from the Arizona Department of Public Safety or eligibility to obtain one within eight weeks of hire (Good Cause Exception must be resolved before the hire date).
  • Pre-employment drug screening required.
  • TB test clearance required at hire
  • Valid Arizona driver’s license with state-minimum insurance coverage and ability to meet Copa Health’s driving requirements.

Nice To Haves

  • Peer Support Training certification preferred or willingness to obtain certification.

Responsibilities

  • Conduct outreach and navigation services in teams of 2-3, engaging with individuals in parks, libraries, faith-based communities, freeway underpasses, and similar areas.
  • Provide information on health, wellness, and prevention services.
  • Assist individuals in completing applications for benefits such as AHCCCS, SSI/SSDI, unemployment, and SNAP, as well as housing services, rental/utility assistance, and employment-related services.
  • Help individuals obtain necessary documentation, including photo IDs, social security cards, and birth certificates.
  • Connect individuals with community-based resources and educate them on available services.
  • Work in unpredictable and fast-paced environments with individuals facing homelessness, legal challenges, and/or mental health or substance use issues.
