About The Position

Brilliant Corners provides innovative housing and housing-related services to California’s most vulnerable individuals, with an emphasis on those transitioning from, or at risk of, homelessness or institutionalization. The Sacramento Flexible Housing Pool (FHP) is a new countywide initiative led by Brilliant Corners in partnership with the Sacramento County Department of Homeless Services and Housing (DHSH). The program provides a centralized system to help individuals experiencing or at risk of homelessness quickly access and maintain permanent housing. In its first year, FHP will serve at least 250 households prioritizing adults experiencing homelessness who also live with serious mental illness (SMI) or substance use disorder (SUD). Through rental subsidies, housing navigation, tenancy supports, and landlord engagement, FHP connects people to stable housing while offering the supports needed to prevent returns to homelessness. By braiding multiple funding streams, including CalAIM, BHSA, PATH CITED, and others, FHP is designed to grow over time into Sacramento County’s primary rehousing pathway, ensuring equitable access to housing and long-term stability for the community’s most vulnerable residents.

Requirements

  • One year of work experience in non-profit human services
  • Excellent verbal and written communication skills
  • Strong interpersonal and de-escalation skills
  • Willingness to travel all over Sacramento County and make regular in-person visits in the community
  • Possession of a valid California driver’s license, a clean driving record, and automobile insurance
  • Access to reliable transportation
  • Basic computer knowledge, MS Word and Excel required
  • Flexibility is required regarding scheduling and prioritizing tasks
  • Ability to utilize critical thinking skills in decision-making situations, good organizational and record-keeping skills, and good independent judgment
  • A valid, clean CA driver’s license and a personal insured vehicle are required.

Nice To Haves

  • Experience working with people experiencing homelessness and/or people with a behavioral health diagnosis a plus
  • Knowledge of different housing models – such as permanent supportive housing or rapid rehousing – or Housing First preferred
  • PowerPoint preferred

Responsibilities

  • Complete housing needs assessment and housing support plan to determine participant’s housing-related needs and goals
  • Work with the Housing Acquisitions team to assist with housing placements and assignments.
  • Coordinate program participants' move-ins into interim housing, scattered-site units, project-based units, and/or residential facilities.
  • Ensure unit habitability through a regular schedule of unit inspections.
  • Assess the need for reasonable accommodations/unit modifications and ADA compliance and assist program participants through the process of obtaining the appropriate information and documentation, if applicable.
  • Provide education on tenant rights and responsibilities as well as life skills related to housing.
  • Routinely make home visits and phone calls to program participants to ensure housing stability.
  • Respond to housing and tenant-related emergencies.
  • Submit and follow up on tenant maintenance requests.
  • Collaborate with case managers, service providers, and property providers to ensure any issues that threaten tenancy are promptly addressed
  • Maintain thorough and accurate progress notes, files, and correspondences while maintaining the confidentiality of program participants, staff, and organizational information at all times.
  • Update multiple data systems with appropriate participant information.
  • Attend all agency staff meetings and trainings, as well as department meetings and case conferences.
  • Other duties as assigned by the Program Supervisor or Program Manager.
