Reporting to the Manager of Housing and Client Care, the incumbent will be responsible for identifying client needs through ongoing assessment and client-directed care planning. This role involves implementing client-centered plans focused on strengths, wellness, and empowerment to support recovery and goal achievement. The Case Manager will collaborate with clients and their Clinical Case Managers to address housing needs, develop, seek out, and recommend potential market-based landlords. The position also includes implementing client-centered approaches to promote housing stability, providing crisis prevention and intervention planning to avoid relapse or hospitalization, and offering short, non-intrusive monitoring support. The program functions under the Housing First model.
