Job Title: Housing Case Manager Department: Coordinated Entry and Supportive Housing Fixed Hourly Wage: $21.50 Position Type: Full Time, Non-Exempt, M-F Direct Supervisor: Supportive Housing Supervisor Work Environment: In person office setting, areas of which are open to the public. May commute for client visits. Job Summary Provide case management to households experiencing homelessness through our Supportive Housing Programs. Case Management includes meeting with, intake, and relationship building with clients in a variety of settings to assist with housing stability within program guidelines. Case management activities include, but are not limited to, establishing stability plans, budgeting, housing search, landlord mediation, document collection, benefit application assistance, crisis intervention, and more. Client barriers may include but are not limited to mental health, substance use disorders, eviction history, unemployment, and medical illnesses. The case manager will collaborate with other service providers in order to ensure each household is connected to the resources they desire for their success.
