About The Position

At Piedmont Crossing, the Household LPN helps create a warm, person-centered environment where residents are empowered to live with dignity, choice, and purpose. Working within a household model of care, the LPN collaborates with the Nurse Leader and interdisciplinary team to provide clinical care and build meaningful relationships with residents and their families.

The role performs nursing duties requiring Practical Nurse licensure, including medication administration, resident assessments and observations, documentation, and communication with physicians, registered nurses, and other healthcare professionals. It also supports regulatory compliance, promotes resident-directed care, and serves as a mentor and role model for professional nursing practices within the household.

Requirements

• Graduate of an accredited School of Practical Nursing.
• At least one year of nursing experience.
• Current Licensed Practical Nurse (LPN) license in North Carolina and ability to maintain licensure in good standing.
• Current CPR certification.
• Demonstrated leadership, communication, and interpersonal skills.
• Knowledge of federal and state regulations related to nursing practice and resident care.
• Commitment to person-centered, resident-directed care and quality outcomes.
• Ability to work collaboratively as part of a household team while supporting continuous learning and excellence.

Nice To Haves

• Experience in a skilled nursing, long-term care, or rehabilitation setting.
• Experience or specialized training in geriatric nursing.

Responsibilities
