About The Position

The Hospitalist is responsible for the provision of inpatient care of Family HealthCare patients in the hospital setting. The promotion of the Patient Centered Health Home through consistent continuity and coordination of care is a priority.

Requirements

  • Equivalent of extensive formal training in Family Medicine or Internal Medicine, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a Doctor of Medicine or Doctor of Osteopathic Medicine degree.
  • Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  • Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
  • Licensed to practice medicine in the state of California.
  • Board-eligibility or certifications may be required or desired in certain positions.
  • Requires the employee to effectively communicate their professional opinions and extrapolations of information they collect and synthesize/analyze.
  • Employees must determine appropriate methods of communicating information through the use of tables, graphs, charts and other visual forms.
  • Duties require the preparation and execution of presentations to large groups.

Responsibilities

  • Provides examination, diagnoses and treatment of hospitalized patients.
  • Manages the patient through the continuum of the hospital experience.
  • Orders treatment, tests, and consults with specialty physicians and primary care physicians when appropriate.
  • Assures continuity of care by communicating with Family HealthCare Network, (FHCN) outpatient providers and support staff regarding admission, status/ progress, discharge, and relevant orders and prescriptions.
  • Participates in medical staff quality improvement and review activities at the hospital and FHCN relevant clinical measures.
  • Participates in appropriate FHCN meetings and staff education.
  • Assures documentation of hospitalized patients in FHCN’s electronic health record in a timely manner.
  • Accountable to and assures a three month schedule is provided to the assigned Medical Director.
  • Will follow relevant and appropriate FHCN policies and procedures as well as assigned Hospital policies and procedures.
  • Assures all billing documentation is submitted accurately and in a timely manner.
  • Accountable for providing care with a high level of customer service to the patient and family.
  • Accountable for patient status as measured by relevant clinical quality measures.
  • Assists with preceptorship of AT Still medical students and/ or residency providers.
  • Meets Network and credentialing expectations for licensure, and employment.
  • Performs other duties as assigned.
