About The Position

Hennepin Healthcare is seeking to fill a Hospital Responder to join our Next Step team. This full-time role will primarily work on-site but also requires a driver's license and access to a reliable and insured vehicle for travel to local hospitals. This role will work Sunday–Thursday from 2:00 p.m.–10:00 p.m. Next Step is a grant-funded department that works as a hospital-based violence intervention program partnership between the City of Minneapolis and Hennepin Healthcare System (HHS). The program serves victims of violent injury (e.g. gunshot, stabbing, serious assault) who have been treated at HHS/HCMC, North Memorial, Children's, or Abbott Northwestern.

Requirements

  • High school diploma/GED or equivalent
  • Valid driver's license
  • Basic computer skills
  • Ability to keep accurate and timely records
  • Ability to read and follow written instructions; understands and communicates verbal and written information
  • Ability to work well with others

Responsibilities

  • Conducts daily hospital rounds connecting with participants still hospitalized; connects with any new participants who have arrived on weekdays since last on-call shift
  • Provides weekday evening and overnight coverage
  • Responds to notifications of eligible participant arrival at Hennepin County Medical Center, North Memorial Hospital, Children's, or Abbott Northwestern Hospital; arrives at hospital within 30 minutes of notification; stays at hospital as needed or until shift ends
  • Conducts initial bedside visit(s) and provides immediate crisis response for patient and family support system
  • Provides comfort and education about emotional consequences following violent injury
  • Builds rapport, assesses risk for retaliatory violence, assesses strengths and needs, and determines need for community-based follow-up services
  • Assesses participant needs and addresses community-based needs, including assistance with employment, education, health care, basic needs, housing, legal aid, transportation, and social and recreational program assistance
  • Guides participant through development and achievement of goal plan
  • Provides referrals to community service providers and fosters connections to long-term resources
  • Supports participant in navigating systems
  • Provides participants with connection to positive, trusted adult
  • Maintains ongoing contact with youth and family until patient is discharged from care to assigned Next Step Case Manager
  • Documents all participant contacts and activities
