We are seeking an experienced Advanced Practice Provider (NP or PA) to join our Hospital Medicine team in a dedicated admissions and triage role. This APP will manage the admissions triage pager and coordinate inpatient admissions from the: Emergency Department (ED), Outside hospitals (OSH) transfers, Direct admissions, and ICU transfers; Evaluate patients in the Emergency Department when there is a clinical concern or when an in-person assessment is warranted; Assist with completing admissions during periods of lower triage volume or when there are breaks in incoming pages; and, Collaborate closely with hospitalists, emergency medicine physicians, nursing staff, and transfer center personnel to facilitate safe and efficient patient placement. Additionally, performance metrics will be monitored to ensure timely, high-quality patient care, including: Length of Stay (LOS), Time to Admit (TAT-A), Response times for direct admissions, and Response times for ICU transfers. Rotation: 7 days on / 7 days off Rotation between the admissions triage service and the observation unit, either: One month on the triage pager followed by one month on the observation team, or One week on the triage pager, one week off, followed by one week on the observation team (based on departmental scheduling).
