About The Position

The Registered Nurse (RN) will be responsible for meeting with potential patients and their families to determine hospice eligibility and provide hospice education in the hospital setting. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community resources. Brighton/Suncrest Hospice aims to change the expectation of hospice care by providing exceptional care and service, achieved by increasing staff to patient ratios, clinical visits, and lowering clinician caseloads. The company is CHAP certified and seeks individuals committed to providing the highest quality of care.

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
• Ability to work independently and manage activities such as phone, email, and written communication.
• Ability to attend meetings with a variety of healthcare professionals.
• Understanding of hospice rules and regulations.
• Time management skills.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Meet with potential patients and their families to determine hospice eligibility.
• Provide hospice education in the hospital setting.
• Provide expertise regarding psych-social assessments and interventions.
• Provide knowledge of community resources.
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1-1 level.
• Manage visits, documentation, and meetings.
• Respond to urgent needs by prioritizing and triaging multiple concerns.
• Travel across the service area.
