About The Position

Suncrest Hospice aims to redefine hospice care by offering exceptional service. This is achieved by increasing staff to patient ratios, which allows for more clinical visits and lower clinician caseloads. Suncrest Hospice is CHAP certified. This role involves providing care and services to patients in their residences, including private homes, Skilled Nursing Facilities, and assisted living facilities. The Resource Nurse is a key member of the interdisciplinary team, offering expertise in psychosocial assessments, interventions, and knowledge of community resources.

Requirements

Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
Ability to work well within a team environment.
Ability to work independently on a day-to-day basis.
Understanding of hospice rules and regulations.
Time management skills.
Ability to learn an electronic medical records system.
Willingness to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area.

Responsibilities

Providing care and services for a group of patients.
Visiting patients in their residences (private homes, SNFs, assisted living facilities).
Serving as a contributing member of the interdisciplinary team.
Providing expertise in psychosocial assessments, interventions, and community resources.
Collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
Working closely with patients on a 1-1 level.
Managing visits, documentation, and meetings.
Responding to urgent needs by prioritizing and triaging multiple concerns.
Learning an electronic medical records system.
