About The Position

The Hospital Discharge Liaison provides quality home care services to seniors and people with disabilities, thereby improving their quality of life and enabling them to remain living at home independently and safely.

Requirements

  • Completion of Homebridge’s Home Care Professional training course
  • At least 6 months of previous home care experience required
  • Basic arithmetic
  • Knowledge of basic domestic chores including cooking and cleaning
  • Able to perform personal care services
  • A sincere desire to help people in need and provide them compassionate care
  • Patient and caring personality
  • Must be dependable and able to follow directions
  • Able to work and travel in all areas of San Francisco
  • Ability to effectively communicate and understand directions

Nice To Haves

  • One year of home care experience highly preferred
  • Experience working with clients who have been previously hospitalized for long periods of time highly preferred
  • Bilingual skills in Spanish, Russian, Cantonese, Japanese, or Tagalog are preferred
  • Experience working with target populations, including people of diverse cultural, education, socio-economic, and linguistic backgrounds, and knowledge of related issues preferred

Responsibilities

  • Provide personal care services including ambulation, bathing, bowel and bladder care, dressing, range of motion exercises, feeding, grooming, assistance with self-administration of medications, menstrual care, prosthesis care, assistance with self-administered oxygen, rubbing skin with lotion, turning client in bed, and paramedical services.
  • Perform domestic services including sweeping, vacuuming, washing floors, cleaning kitchen and bathroom, storing food and supplies, taking out garbage, dusting, changing bed linen, menu planning, preparing meals, meal clean-up, laundry services, and reasonable shopping and errands.
  • Accompany clients to appointments with doctors, dentists, and other health care practitioners, and for fittings for health-related appliances and special clothing, using public transportation or taxicabs if necessary.
  • Perform heavy cleaning when authorized by a supervisor, which may include deep cleaning tasks, removing hazardous debris and dirt, thoroughly cleaning cupboards, cleaning beneath and behind furniture, and other approved domestic chores.
  • Report changes in the client’s health or safety to the supervisor.
  • Recommend ways to improve a client’s safety and comfort to the supervisor.
