The Hospice Support Manager provides operational and clinical leadership for hospice support services, overseeing the intake, triage, admissions, float nurse support, liaison, and ancillary staff. This role ensures timely, accurate, and patient-centered Hospice and Palliative Care. This role supervises support staff, facilitates coordination of care activities, and ensures compliance with agency policies, regulatory requirements and quality standards. The position serves as a leadership resource for staff, patients, and families, supporting day-to-day operations, staff development, and continuity of care. While primarily managing, the Hospice Support Manager must provide direct patient care to meet patient, staff, and agency needs. This role includes participation in leadership, staff training, quality improvement initiatives, and the administer on-call rotation. Reliable travel to patient homes, facilities, and community locations is required. The Hospice Support Manager collaborates closely with the interdisciplinary team and leadership to promote timely patient admission to Tri-Cities Chaplaincy services, efficient workflow, high-quality patient care, and a supportive environment for staff and families.
