As a Spiritual Coordinator, you’ll contribute to our success in the following ways: Conduct initial and ongoing spiritual assessments to identify the needs, beliefs, and preferences of hospice patients and families. Develop individualized spiritual care plans and collaborate with the interdisciplinary team to integrate spiritual needs into overall patient care. Provide spiritual guidance, counseling, and emotional support while respecting each patient’s cultural and religious beliefs. Support patients and families with end-of-life concerns, including discussions around meaning, faith, forgiveness, and personal values. Provide bereavement support to families before and after a patient’s passing. Educate patients, families, and staff on spiritual care and maintain accurate, timely documentation of services provided.

Requirements

  • Bachelor’s or Master’s degree in divinity, Theology, Pastoral Care, or a related field
  • 2 years of experience working at a hospice, counseling, or ministry environment
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations

Responsibilities
