About The Position

Advantage Hospice is seeking a dedicated and empathetic Hospice Social Worker to join its interdisciplinary team. The role provides compassionate, holistic social work services to hospice patients and their families, including emotional guidance, spiritual support, crisis counseling, advocacy, and connections to community resources. The position requires flexibility, compassion, and commitment, and duties may evolve as needed.

Requirements

• One of the following education and experience combinations: an MSW from a CSWE-accredited school and one year of health care experience; a BSW from a CSWE-accredited school and one year of experience in a health care setting, supervised by an MSW unless exempt; or a bachelor's degree in psychology, sociology, or a related field and one year of health care social work experience with MSW supervision.

• Ability to sit, stand, bend, and lift light loads regularly.

Responsibilities

• Perform initial and ongoing psychosocial, spiritual, and emotional assessments.
• Collaborate in developing patient-centered care plans.
• Provide individual and family counseling, including crisis intervention.
• Serve as a bereavement team member and offer grief support as needed.
• Advocate for patients and families in IDG meetings.
• Maintain accurate and timely documentation of services and contacts.
• Develop community relationships to enhance referrals and outreach.
• Participate in staff meetings and continued professional development.
