About The Position

AdvisaCare Home Health and Hospice is hiring a full-time MSW Social Worker to serve the Genesee and Lapeer County areas, with the Troy location as the parent agency. The role supports patients with life-limiting illnesses and their families through hospice care. The social worker provides care under physician direction and participates in interdisciplinary team conferences and the Bereavement Team as assigned.

Requirements

Master of Social Work degree from a school of social work accredited by the Council on Social Work Education (CSWE).
Valid driver's license, car insurance, and reliable transportation.

Nice To Haves

One year of home health care experience preferred.
Hospice experience preferred.

Responsibilities

Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist with developing and implementing goal-directed care plans.
Participate in interdisciplinary team conferences, observing and assessing conditions affecting patients and families and sharing relevant information.
Document patient and family visits, telephone contacts, and referral actions promptly, accurately, and concisely in patient records.
Reassess patient and family needs and provide counseling as required.
Provide crisis intervention and individual or family counseling when indicated.
Act as a care advocate for the patient and family unit.
Develop and maintain contacts with community agencies and services to promote cooperation and facilitate referrals.
Provide ongoing counseling to patients and families about death and dying as needed.
Participate as a member of the Bereavement Team when assigned.
Adhere to hospice policies.
