About The Position

Join our team for a 13-week Hospice RN Case Manager contract where compassion meets expertise. You’ll provide exceptional hospice care to patients and families while coordinating individualized care that supports comfort, dignity, and quality of life. This is a contract nursing position. The selected RN must be experienced, confident, and prepared to begin managing patients with limited orientation.

Requirements

  • Graduate of an accredited school of nursing.
  • Current, active RN license in good standing in South Carolina.
  • At least one year of hospice-care experience.
  • Must be able to work independently and transition quickly into the role with limited orientation.
  • Strong clinical documentation skills with excellent attention to detail.
  • Tech-savvy and comfortable learning and navigating electronic medical record systems.
  • Current CPR certification.
  • Valid driver’s license, reliable transportation, and proof of automobile liability insurance.

Nice To Haves

  • One year of precepting or training experience is preferred.

Responsibilities

  • Maintain an assigned hospice case-management load.
  • Complete initial and comprehensive patient and family assessments to determine hospice needs.
  • Assess current and previous illnesses and perform physical assessments, emphasizing pain and symptom management.
  • Complete ongoing assessments of the patient’s physical condition and the psychosocial needs of the patient and family.
  • Develop individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patient, family, and attending physician.
  • Revise the plan of care as the patient’s or family’s condition and needs change.
  • Coordinate care among IDG members and external healthcare providers.
  • Evaluate and coordinate services according to the established plan of care.
  • Direct and supervise services provided by assigned LPNs/LVNs, Hospice Aides, and Homemakers.
  • Facilitate transitions to other levels of care, including inpatient, respite, or continuous care, when appropriate.
  • Participate in on-call duties as defined by the office’s on-call policy.
  • Complete timely, accurate, and thorough clinical documentation.
  • Access, use, and disclose protected health information as necessary to fulfill assigned responsibilities and in accordance with organizational policies.
