About The Position

Interim HealthCare is seeking an experienced Home Health Occupational Therapist for a 13-week, full-time contract. In this role, you’ll provide personalized therapy that helps patients regain their independence and safely perform everyday activities in the comfort of their homes. This is a contract position requiring at least one year of home health experience. The selected OT must be confident, independent, and prepared to manage patients with limited orientation.

Requirements

  • Graduate of an accredited Occupational Therapy program.
  • Current, active Occupational Therapist license in good standing in South Carolina.
  • At least one year of occupational therapy experience in a home health setting is required.
  • Must be able to work independently and transition into the role with limited orientation.
  • Strong documentation and time-management skills.
  • Tech-savvy and proficient in navigating electronic medical record systems.
  • Knowledge of state and federal home health regulations.
  • Strong clinical judgment, interpersonal skills, and problem-solving abilities.
  • Current CPR certification.
  • Valid driver’s license, reliable transportation, and automobile insurance.
  • Compassionate, dependable, and resourceful.

Responsibilities

  • Provide occupational therapy to patients who have difficulty performing daily activities due to an illness or injury.
  • Perform comprehensive assessments, identify functional deficits, establish therapy goals, and document patient progress.
  • Develop and implement individualized treatment plans based on each patient’s needs and goals.
  • Work independently while collaborating with the home health team, which may include RNs, LPNs, CNAs, HHAs, PTs, and SLPs.
  • Help patients improve fine motor skills, strength, mobility, and coordination.
  • Recommend adaptive equipment, including grab bars, shower chairs, and other supportive devices.
  • Assess fall risks and implement strategies to improve safety within the home.
  • Educate patients and families about the plan of care, exercises, goals, safety, and self-care.
  • Complete accurate, thorough, and timely clinical documentation.
  • Provide services in accordance with agency policies and all applicable regulations governing the scope of practice.
