Requirements

Current RN or clinical licensure in the state of Ohio required
Prior experience in hospice, home health, or palliative care quality assurance or compliance
Strong knowledge of CMS hospice Conditions of Participation and QAPI regulations
Experience with clinical auditing, data analysis, and performance improvement methodologies
Excellent organizational, communication, and critical thinking skills
Proficiency with electronic health records (EHR) and quality reporting tools
Compassionate, detail-oriented, and committed to delivering exceptional end-of-life care

Nice To Haves

Prior management experience in hospice, home health or pallaitive care a plus

Responsibilities

Develop, implement, and oversee the Quality Assurance and Performance Improvement (QAPI) program in compliance with hospice regulations and standards
Monitor and analyze clinical and operational data to identify trends, gaps, and opportunities for improvement
Lead performance improvement initiatives and coordinate interdisciplinary team involvement in quality projects
Conduct audits of clinical documentation, patient records, and care plans to ensure regulatory compliance
Prepare and present quality reports and metrics to leadership and clinical staff
Ensure adherence to CMS Conditions of Participation and state hospice regulations
Collaborate with clinical staff to develop and deliver education related to quality standards and best practices
