About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Hospice RN On-Call provides after-hours clinical support, triages patient needs, and coordinates care to ensure continuity and quality outcomes. The role delivers remote and field-based nursing support, assesses patient conditions, and collaborates with interdisciplinary teams to address urgent and emergent needs. The position includes providing patient and caregiver education, documenting clinical interactions and decisions, and coordinating with physicians and care teams. The role offers opportunities for development and is compensated based on factors including local labor markets, education, work experience, and certifications. The stated annual salary range is $100,722 to $151,048 based on full-time employment.
Requirements
- Current RN license specific to the state(s) assigned for work.
- At least 1 year of nursing experience; hospice, home health, or acute care experience is preferred.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and liability insurance.
- Candidates must pass a drug test before beginning employment.

Responsibilities
- Provide on-call nursing support, triage patient needs, and determine appropriate interventions.
- Assess patient conditions by phone and/or in-person visits and escalate care as needed.
- Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
- Educate patients and caregivers about symptom management and care plans.
- Maintain accurate and timely documentation of patient interactions and clinical decisions.
