Position Summary The Hospice RN Case Manager serves as the clinical leader for an assigned patient caseload, coordinating and managing all aspects of hospice care. This role requires clinical excellence, strong organizational skills, sound judgment, and a caring heart. As the primary point of contact for patients and families, the RN Case Manager ensures that each patient's plan of care reflects their goals, values, and changing needs while providing education, comfort, advocacy, and support throughout their hospice journey. This position requires someone who embraces ownership and accountability, works independently with confidence, and collaborates seamlessly with our interdisciplinary team to provide exceptional, patient-centered care.

Requirements

  • Current RN license in the Commonwealth of Massachusetts.
  • Graduate of an accredited nursing school.
  • Hospice or home health nursing experience preferred.
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Valid driver's license and reliable transportation.
  • Ability to manage multiple priorities while providing compassionate, individualized care.
  • Experience with electronic medical records preferred.

Responsibilities

  • Manage and coordinate comprehensive hospice care for an assigned patient caseload.
  • Perform skilled nursing assessments and ongoing evaluations of patient needs.
  • Develop, implement, and update individualized plans of care.
  • Provide symptom management, medication education, and end-of-life nursing care.
  • Educate patients, families, and caregivers with compassion and patience.
  • Coordinate services with physicians, facilities, and community providers.
  • Collaborate closely with our interdisciplinary team, including: Social Workers, Home Health Aides, Chaplains, Music Therapists, Hospice Physicians, Business Office Staff, Clinical Directors.
  • Participate actively in interdisciplinary team meetings and care planning.
  • Maintain accurate, timely clinical documentation.
  • Advocate for patients and families while promoting comfort, dignity, and quality of life.
  • Participate in a shared after-hours on-call rotation as appropriate.
