About The Position

Care Initiatives Hospice is seeking a compassionate Registered Nurse to serve as a Hospice RN Case Manager. The role provides individualized hospice care to patients in their places of residence, which may include private homes, long-term care communities, assisted living residences, or hospitals. Care is holistic and addresses patients’ physical, emotional, and spiritual needs while working collaboratively with families, caregivers, physicians, and the interdisciplinary team to honor patients’ needs and wishes.

Requirements

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Current driver's license and automobile liability insurance, and ability to travel between multiple locations within the region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.

Nice To Haves

• Two years of verifiable experience working as a nurse (RN/LPN) is preferred.

Responsibilities

• Use clinical nursing knowledge, physical assessment, and procedural skills to provide high-quality patient care in the patient’s place of residence.
• Develop and oversee the plan of care under physician direction and ensure appropriate interdisciplinary involvement.
• Perform initial and comprehensive patient assessments in accordance with policies and procedures.
• Conduct ongoing visits and interdisciplinary assessments of patients.
• Provide quality care and complete clear, concise, and accurate clinical documentation.
• Counsel and educate patients, caregivers, and families as indicated in the plan of care.
• Develop and continually update plans of care to identify the services required to meet each patient’s needs.
