About The Position

As an RN Case Manager, you will coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The role includes patient and caregiver education, accurate and timely care documentation, communication with the clinical manager and care team, and on-call responsibilities as assigned. Amedisys is part of the Optum family of businesses and provides home health, hospice, and community-based services.

Requirements

• Current RN license specific to the state(s) assigned.
• At least 1 year of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately and in a timely manner according to care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Perform other duties as assigned.
