About The Position

Sage Group HealthCare Staffing is seeking a Hospice RN Case Manager for a contract position in Lebanon, NH. This role involves screening, evaluating, re-evaluating, and treating patients, as well as coordinating and communicating treatment activities with the interdisciplinary team and the patient. The RN Case Manager will develop and periodically review a care plan in collaboration with the patient's physician, patient, and caregivers. Responsibilities include coordinating the care plan through interdisciplinary communication, assessing the patient's clinical, functional, and emotional status, communicating significant changes to the physician and colleagues, documenting evaluations and treatment goals, teaching self-care techniques, and providing medication and diet instructions.

Requirements

Registered Nurse: 1-2 years - Required
Hospice Nursing experience: 1-2 years – Required
Must have valid Driver’s License and Personal Auto Insurance
ASN or an equivalent – Required
RN License for State/s applying or Compact - Required
BLS Certification from AHA or American Red Cross - Required

Nice To Haves

EMR Epic - Preferred
BSN or an equivalent – Highly preferred

Responsibilities

Screen, evaluate, re-evaluate, and treat patients.
Coordinate and communicate treatment activities with the interdisciplinary team members and the patient.
Develop a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission.
Periodically review the patient care plan and recertify the patient care plan as appropriate.
Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate.
Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals.
Communicate significant changes to the patient's physician and colleagues.
Document evaluations, treatment goals, and objectives.
Teach the patient and caregiver self-care techniques as appropriate.
Provide medication, diet, and other instructions as ordered by the physician and utilizes opportunities for health counseling with patients and caregivers.
