About The Position

Hospice RN Case Manager North and NE Columbus area Mount Carmel Home Care & Hospice | Columbus, Ohio At Mount Carmel Home Care & Hospice, we believe exceptional care begins with exceptional people. We're seeking a compassionate and skilled Hospice RN Case Manager to join our dedicated team serving patients and families throughout the Columbus area. This is an opportunity to provide meaningful end-of-life care while working alongside a supportive interdisciplinary team committed to dignity, comfort, and quality of life for every patient. Position Summary The Hospice RN Case Manager is responsible for coordinating and managing the care of hospice patients and families. This role serves as the primary clinical resource, developing individualized care plans, managing symptoms, coordinating interdisciplinary services, and providing education and emotional support throughout the hospice journey. The RN Case Manager also provides functional supervision and direction to Hospice Aides and LPNs while ensuring quality patient outcomes and regulatory compliance.

Requirements

  • Graduate of an accredited Nursing program.
  • Current Ohio Registered Nurse (RN) license.
  • Current CPR certification.
  • Minimum one (1) year of acute care nursing experience.
  • Valid driver's license and reliable transportation.
  • Excellent communication, assessment, and critical thinking skills.

  • Knowledge of hospice and palliative care principles.

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized plans of care.
  • Coordinate care with physicians, social workers, chaplains, aides, and other team members.
  • Manage pain and symptoms using evidence-based hospice and palliative care practices.
  • Provide education and support to patients, families, and caregivers regarding disease progression and end-of-life care.
  • Monitor patient status and communicate changes promptly to physicians and the interdisciplinary team.
  • Participate in IDG/IDT meetings and collaborate on patient care planning.
  • Ensure accurate and timely documentation in the electronic medical record.
  • Coordinate referrals, recertifications, supplies, and patient care services.
  • Supervise Hospice Aides and LPNs to ensure quality care delivery.
  • Promote dignity, comfort, and quality of life for hospice patients and their families.
