About The Position

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and dedicated RN Hospice Case Manager to serve patients and families across Southern Palm Beach County. The role provides comfort, dignity, and support to individuals facing advanced illness as part of a collaborative, mission-driven team. The RN Case Manager coordinates care and supports patients and families throughout their hospice journey. The position works under the direction of the primary physician and in collaboration with the interdisciplinary team to develop and implement individualized plans of care. The schedule is 8:30 a.m. to 5:00 p.m., including every other weekend.

Requirements

Current Registered Nurse license in the State of Florida.
Current Basic Life Support certification.
Understanding of and ability to apply hospice nursing characteristics, including case management, crisis intervention, communication, teaching, thanatology, and needs assessment skills.
Adequate and reliable transportation, adequate insurance, and a valid Florida driver's license.

Nice To Haves

One year of experience in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or hospice nursing.
Basic computer skills and experience with EMR software.

Responsibilities

Advocate for patients and families, ensuring their needs are recognized and addressed.
Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team.
Develop and implement individualized plans of care as part of the Interdisciplinary Group (IDG).
Monitor patient conditions, provide interventions, and deliver education to enhance comfort and quality of life.
Address concerns promptly and thoughtfully to support satisfaction and trust.
