About The Position

The RN Case Manager is responsible for performing visits and completing the coordination of client care. This role serves as the point of contact for all disciplines involved in patient care and oversees the frequency of visits for the episode. The RN Case Manager consults with the physician and the office regarding patient care, performs accurate OASIS collection, ensures the medication profile is current, and reports lab values to the physician in a timely manner. Key responsibilities also include attending weekly case conferences and monthly case manager meetings. All patient care must be provided with respect for patient rights, dignity, and expressed needs. This role involves assessing the patient’s physical, psychological, and social needs, assessing pain and implementing appropriate pain management techniques, and documenting patient history, symptoms, assessments, tests, and care according to guidelines. Reviewing patient history, diagnostics, and lab data, and reporting any abnormalities are also crucial aspects of this position.

Requirements

  • Current, unencumbered RN or LPN license
  • Certification in CPR
  • Prior clinical experience
  • Excellent communication skills
  • Demonstrated ability to maintain a high level of professionalism during stressful times
  • Strong critical thinking skills
  • Ability to adapt to different clinical settings and environments
