About The Position

The Hospice Registered Nurse Case Manager delivers professional nursing care to the agency’s home care patients as directed by the physician and in accordance with the State Nurse Practice Act and agency policies and procedures. Additionally, the RN may provide nursing services in an institutional setting while adhering to that institution’s policies and procedures.

Requirements

  • Must hold a current Registered Nurse (RN) license in the State of Oklahoma.
  • A minimum of two years of hospice nursing experience is preferred.
  • Certification in Hospice and Palliative Nursing is highly desirable.
  • Strong verbal, written, and electronic communication skills.
  • Ability to adapt to frequently changing tasks and priorities.
  • Must possess a valid driver’s license, have reliable transportation, maintain active auto liability
  • Excellent attention to detail and the ability to focus despite frequent interruptions.

Nice To Haves

  • Certification in Hospice and Palliative Nursing is highly desirable.

Responsibilities

  • Provide direct nursing care to assigned hospice patients across various settings, ensuring their needs and goals are met.
  • Execute all RN responsibilities in alignment with the hospice plan of care.
  • Conduct patient admissions, routine skilled nursing and supervisory visits, hospice evaluations, imminent visits, on-call duties, and PRN visits as assigned.
  • Maintain and update patient medical records in compliance with agency standards.
  • Work collaboratively within an interdisciplinary team, maintaining clear role boundaries and effective teamwork.
  • Participate in interdisciplinary team meetings and case presentations.
  • Engage in ongoing education and training to stay current on treatments, procedures, and best practices.
  • Demonstrate a deep understanding of death and dying, pain and symptom management, and grief and loss.
  • Conduct supervisory visits to assess and reevaluate patients' nursing needs.
  • Perform home health aide supervisory visits as assigned.
  • Provide monthly supervision for any assigned Licensed Practical Nurses (LPNs).
  • Support and deliver care across all levels of hospice services, including routine, respite, continuous, inpatient, and imminent visits.
  • Ensure accurate, timely, and complete documentation that meets agency and Medicare standards.
  • Attend interdisciplinary meetings every 15 days.
  • Participate in case conferences and care coordination meetings with healthcare partners, including nursing homes, assisted living facilities, and hospitals, ensuring seamless patient care.
  • Serve as a case manager, overseeing the entire hospice care process and accommodating irregular hours as necessary.
  • Adhere to all policies, procedures, and supervisory expectations as directed by leadership.
  • Submit required documentation promptly to maintain compliance with personnel file requirements.
  • Respond to emergencies and remain on standby until able to contact a supervisor or manager.
  • Report any changes in a patient’s condition to the appropriate team member and ensure timely follow-up.
  • Communicate effectively with care coordination teams, including facility staff, physicians, pharmacies, case teams, and supervisors.
  • Educate patients and caregivers on disease processes, care goals, and rehabilitation per Medicare guidelines and agency policies.
  • Implement appropriate preventive and rehabilitative nursing interventions.
  • Participate in the on-call rotation, including weekends and holidays, as required.
  • Train and mentor other staff in hospice care procedures.
  • Represent the agency in meetings as needed.
  • Assist in onboarding and training new employees as requested.
  • Uphold ethical principles, including acceptance and non-judgmental patient care.
  • Adhere to all laws, regulations, and workplace policies governing hospice care and non-discriminatory practices.
