About The Position

Constellation Health Services is seeking a compassionate, skilled Hospice Registered Nurse Case Manager to care for hospice patients and their families in homes, assisted living facilities, and skilled nursing facilities. The role delivers patient-centered care in community settings and coordinates care with the patient’s family, primary caregivers, and other disciplines. The position is based in Marlton, New Jersey, with travel of up to 25%. Weekend and holiday coverage may be needed.

Requirements

Graduate of an accredited RN program.
Registered Nurse licensed in the state of employment.
At least 1–2 years of RN experience.
Basic knowledge of the iOS operating system.

Nice To Haves

Previous hospice care experience is strongly desired.

Responsibilities

Provide skilled nursing care, including medications and treatments prescribed in the physician’s plan of care, to patients in their home setting.
Complete comprehensive assessments and coordinate the plan of care with disciplines involved in meeting the patient’s needs, including family and primary caregivers.
Observe and evaluate patients’ physical and emotional status, and report changes to the nursing supervisor and attending physician.
Participate in weekly Interdisciplinary Team Meetings and provide updates on patients’ physical, emotional, psychosocial, and spiritual status.
Maintain and process documentation in compliance with agency standards.
Use a computer-based documentation system and submit clinical documentation, time records, and other required information within the required time frame.
Provide input to the Home Health Aide Supervisor as needed for hospice aide evaluation.
Provide weekend and holiday coverage as needed.
