About The Position

Jefferson Health at Home by BAYADA is recruiting a compassionate Hospice Registered Nurse (RN) Case Manager to serve patients in small territories throughout Montgomery County and Bucks County, Pennsylvania. The role is part of a multidisciplinary healthcare team providing palliative care so clients can receive needed medical care at home.

The nurse makes home visits within assigned geographic territories and provides medication administration, wound care, treatments, procedures, and comfort care. The nurse monitors client conditions, reports changes to the Clinical or Client Services Manager, and follows up on, executes, and documents doctors’ orders. Occasional after-hours and/or weekend on-call work may be required. Scheduling is primarily during daytime hours.

Requirements

• Hold an active, current license to practice as a professional registered nurse (RN) in Pennsylvania.
• Be able to travel to assigned cases.
• Have a calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
• Deliver patient care with compassion, excellence, and reliability.

Nice To Haves

• One year of hospice experience is strongly preferred.
• Previous home healthcare experience is welcomed but not required.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medication and provide wound care, treatments, procedures, and comfort care.
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
• Follow up on, execute, and properly document doctors’ orders.
• Occasionally provide after-hours and/or weekend on-call coverage.
• Provide 1:1 direct client care and advocate for clients’ needs.
