Hospice RN Case Manager (RN)

As a Hospice RN Case Manager, you’ll deliver skilled nursing care in the comfort of patients’ homes, creating individualized care plans, administering medications, and educating patients and families. Requirements
- Active RN license in Michigan
- 1+ years of clinical nursing experience (Hospice preferred)
- Strong communication, assessment, and organizational skills
- Compassionate, dependable, and mission-minded
- Valid driver’s license and reliable transportation required
