About The Position

The RN Hospice Case Manager is responsible for coordinating and managing the plan of care for hospice patients across all care settings. This role provides comprehensive nursing assessment, care coordination, symptom management, patient and family education, and collaboration with the interdisciplinary team to ensure patients receive compassionate, high-quality end-of-life care. The RN Case Manager serves as the primary clinical leader for assigned patients, promoting comfort, dignity, and quality of life while supporting patients and families through the hospice journey.

Requirements

  • Current, active Registered Nurse (RN) license in good standing in Ohio.
  • Minimum of two (2) years of nursing experience.
  • Valid driver's license, reliable transportation, and automobile insurance.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, organization, and time management abilities.
  • Ability to work independently while collaborating effectively within an interdisciplinary team.

Nice To Haves

  • Previous hospice, home health, palliative care, oncology, or end-of-life care experience.
  • Case management experience.
  • Knowledge of CMS hospice Conditions of Participation.
  • Experience with electronic medical record (EMR) systems.
  • BSN preferred.
  • CHPN certification or willingness to obtain certification.

Responsibilities

  • Conduct comprehensive nursing assessments upon admission and throughout hospice services.
  • Develop, implement, and update individualized plans of care based on patient needs and goals.
  • Manage an assigned hospice caseload and coordinate services across the continuum of care.
  • Monitor patient condition and respond appropriately to changes in clinical status.
  • Provide symptom management and pain management interventions consistent with hospice philosophy and physician orders.
  • Coordinate admissions, recertifications, transfers, discharges, and transitions in care.
  • Participate in after-hours and on-call responsibilities, as assigned.
  • Collaborate with physicians, nurse practitioners, hospice aides, social workers, chaplains, volunteers, and other team members to ensure coordinated patient care.
  • Participate in Interdisciplinary Group (IDG) meetings and care planning discussions.
  • Communicate changes in condition and care needs to appropriate team members.
  • Educate patients and families regarding disease progression, symptom management, medications, and available hospice services.
  • Provide emotional support and guidance through end-of-life care decisions.
  • Promote patient dignity, autonomy, and quality of life in accordance with hospice philosophy.
  • Maintain accurate, timely, and complete clinical documentation consistent with hospice regulations and agency standards.
  • Ensure documentation supports eligibility, recertification, care planning, and reimbursement requirements.
  • Participate in quality assurance, performance improvement, and regulatory compliance initiatives.
  • Support survey readiness and adherence to CMS, state, and federal hospice regulations.
