About The Position
Altus Hospice is seeking a PRN Hospice Registered Nurse Case Manager to provide end-of-life care in Corpus Christi and surrounding areas. The role delivers hands-on hospice nursing care, coordinates services, and guides patients and families throughout their hospice journey as part of an interdisciplinary care team. Care is grounded in compassion, clinical excellence, and connection, with a focus on patient comfort, quality of life, dignity, and support. The schedule is PRN, as needed. The listed average pay range is $35.00–$42.00 per hour; final compensation depends on relevant experience, qualifications, and the position’s specific responsibilities and is confirmed at the conclusion of the interview process.
Requirements
- Hold a current, unencumbered Registered Nurse license.
- Have at least one year of RN experience in a medical-surgical or acute-care setting.
- Be proficient in typing and computer programs, including having an EMR background.
- Maintain current CPR certification if required by the state.
- Have a current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities
- Respond to clinical referral information and address inquiries and requests with care and expertise.
- Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
- Provide direct nursing care and support to patients, prioritizing comfort and quality of life.
- Deliver nursing care using proven principles and techniques to help ease pain and provide comfort.
- Educate and advocate for patients and families, encouraging their participation in personalized care planning.
- Share responsibility for call duties.
