About The Position

Optimal Care is proactively building a talent pipeline for future Hospice RN Case Manager opportunities serving Central and Southeast Michigan. This role involves providing skilled nursing care and compassionate support to patients and families during life's final chapter, ensuring comfort, dignity, and quality of life. The position is ideal for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships with patients, families, physicians, facilities, and other interdisciplinary team members. The role offers autonomy in coordinating comprehensive care for a caseload and requires collaboration with the interdisciplinary team to address physical, emotional, and spiritual needs.

Requirements

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum one (1) year experience as a nurse in a clinical care setting
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Nice To Haves

  • Bachelor of Science in Nursing (BSN)
  • Hospice or home health experience
  • Nursing skills as defined by accepted nursing standards
  • Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment
  • Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
  • Ability to handle multiple priorities, documentation requirements and deadlines
