About The Position

Hospice RN Case Manager Small Caseloads, Meaningful Relationships, Extraordinary Lifestyle Balance At Grand County Hospice, a program of Moab Regional Hospital, we provide what our team calls boutique hospice care —deeply personal, relationship-centered care that allows nurses the time and flexibility to truly support patients and families during one of life's most important transitions. Unlike many hospice programs where nurses carry large caseloads and spend much of their day rushing between visits, our nurses are given the capacity to be fully present with patients and families while providing exceptional end-of-life care. This is a salaried, full-time, benefited position, eligible for overtime. Schedule: Flexible four-day schedule with 10-hour shifts (Monday-Friday) with rotating evening and weekend on-call responsibilities. Salary Range: $68,473 – $97,510 annually, DOE $10,000 Signing Bonus | Relocation Assistance Available | Potential Housing Assistance Available Position Summary The Hospice RN Case Manager coordinates and provides compassionate care for terminally ill patients and their families in home and community settings. This position works closely with physicians, caregivers, and the hospice interdisciplinary team to ensure patients receive coordinated, high-quality care focused on comfort, dignity, and quality of life.

Requirements

  • Current Utah RN license or Compact RN license
  • Graduate of an accredited nursing program
  • Minimum one-year recent RN experience
  • Current BLS certification
  • Valid driver's license and reliable transportation
  • Ability to work independently and provide excellent patient-centered care
  • Compassion for patients and families facing end-of-life challenges

Nice To Haves

  • Hospice or home health experience is preferred but not required.

Responsibilities

  • Conduct patient visits, assessments, and care planning
  • Provide education and support for patients and caregivers
  • Coordinate care with physicians and interdisciplinary team members
  • Arrange medications, equipment, and support services
  • Participate in hospice team meetings and care conferences
  • Provide rotating evening and weekend on-call coverage
  • Maintain accurate and timely clinical documentation
