About The Position

Elara Caring is seeking a compassionate Registered Nurse Case Manager to provide hospice care and support patients, families, caregivers, and facility staff. The role contributes to the company’s mission of delivering the right care at the right time in the right place. The Case Manager assesses hospice patients and families, monitors patient condition, supports implementation of the interdisciplinary Plan of Care, provides hospice-related education, and offers support at the time of death.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Experience in a hospice or home health environment is preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
