PRN Hospice RN - Orange, CT
About The Position
Constellation Health Services is seeking a compassionate and skilled per diem/PRN Hospice Registered Nurse to care for hospice patients and their families in homes, assisted living facilities, and skilled nursing facilities. The nurse covers on-call staff in one of the Orange, Danbury, or Waterbury locations. Coverage is available Monday through Friday from 5 p.m. to 8 a.m., at a $450 flat rate including documentation and travel/mileage, plus $115 per visit. Saturday through Sunday coverage is from 8 p.m. to 8 a.m., at a $600 flat rate including documentation and travel/mileage, plus $115 per visit. Weekend and holiday coverage may be needed. Requirements
- Graduate of an accredited registered nurse program.
- Hold a Registered Nurse license in the state of employment.
- At least 1–2 years of registered nurse experience.
- Basic knowledge of the iOS operating system.
Nice To Haves
- Previous hospice care experience is strongly desired.
Responsibilities
- Provide skilled nursing care, including administering medications and treatments prescribed in the physician’s plan of care.
- Complete comprehensive patient assessments and coordinate the plan of care with relevant disciplines, family members, and primary caregivers.
- Observe, evaluate, and report changes in patients’ physical or emotional status to the nursing supervisor and attending physician.
- Participate in weekly interdisciplinary team meetings and provide updates on patients’ physical, emotional, psychosocial, and spiritual status.
- Maintain and process documentation in accordance with agency standards.
- Use a computer-based documentation system and submit required clinical documentation, time records, and other information within required time frames.
- Provide input to the Home Health Aide Supervisor as needed for hospice aide evaluations.
- Provide weekend and holiday coverage as needed.
- Cover on-call registered nurses in one of the Orange, Danbury, or Waterbury locations.
