About The Position

Residential Home Health and Hospice is hiring a Registered Nurse for a hospice position providing end-of-life care to patients in their homes in the Franklin Park, Illinois area. The role delivers compassionate, patient-centered care focused on comfort, dignity, and quality of life, in collaboration with an interdisciplinary team. The nurse works independently while receiving support from a dedicated field education team and leadership. The position offers a first-year success bonus of $10,000 for accomplishing key milestones during the first year.

Requirements

Active Registered Nurse (RN) license in the state where the employee works.
Two years of RN experience and the clinical expertise to deliver quality patient care.
Valid driver's license.
Ability to spend approximately 20% of the workday driving to and from patient locations.

Responsibilities

Deliver compassionate, patient-centered care in a hospice setting while collaborating with an interdisciplinary team.
Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning.
Educate and support patients and families throughout the end-of-life journey.
Think critically and solve problems in real time.
Maintain thorough, compliant EMR documentation.
Travel to patient homes within the assigned territory.
