About The Position

HopeHealth is seeking a Registered Nurse for its affiliate HopeHealth Hospice & Palliative Care on the Massachusetts hospice team. The RN is assigned a caseload by the Patient Care Manager according to census needs and may cover for an RN Case Manager’s absence to help maintain continuity of care.

This is a part-time, 24-hour day position with a schedule of 8:00 a.m. to 4:30 p.m. The position is eligible for benefits. The RN provides hospice nursing care, assesses patient and family needs, implements and updates individualized plans of care, educates patients and caregivers, and coordinates services with the interdisciplinary team.

Requirements

• Registered Nurse (RN) required.
• 1–3 years of related experience.
• Strong nursing assessment, clinical, and communication skills.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Home care and/or related experience preferred.

Responsibilities

• Conduct initial and ongoing assessments of how the terminal diagnosis affects the patient's physical, functional, psychosocial, and environmental needs and activities of daily living.
• Implement the individualized plan of care, recommend revisions as needed, and plan for discharge or transfer when appropriate.
• Consult with and educate patients, families, and caregivers about the disease process and self-care techniques, and provide physician-ordered medication, diet, and other instructions.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Coordinate patient and family services and prioritize needs with interdisciplinary team members to develop and revise the individualized plan of care.
• Inform the physician and other personnel about changes in patient needs and intervention outcomes, and update the plan of care as needed.
• Determine the scope and frequency of services based on acuity and patient and family needs.
• Coordinate care with interdisciplinary team members and the family or caregiver to address identified psychosocial, spiritual, and emotional needs, and refer to appropriate disciplines or services.
• Provide continuity of care by covering for an RN Case Manager's absence when needed.
