About The Position

Altus Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join their dedicated nursing team, providing exceptional end-of-life care throughout the community. In this role, the RNCM will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey. The position is part of an interdisciplinary care team focused on providing compassionate, clinically excellent, and connected care. Requirements

  • Current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
  • Computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.
  • Current driver's license, vehicle insurance, and access to dependable transportation.

Nice To Haves

  • Hospice experience is a plus.

Responsibilities

  • Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
  • Partner closely with the Clinical Director to assess the eligibility and suitability of clients for specialized hospice services.
  • Deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey.
  • Apply nursing skills by delivering top-notch care rooted in proven principles and techniques to ease pain and bring solace.
  • Empower patients and families through education, encouraging their active participation in creating personalized care plans that align with their goals.
  • Share the responsibility of call duties.
