In this role, you’ll provide patient-centered, holistic care to individuals facing life-limiting illnesses—supporting not just their medical needs, but their emotional and spiritual well-being. You’ll work independently in the field while collaborating closely with an interdisciplinary care team to ensure comfort, dignity, and quality of life.

Requirements

Active RN license
CPR certification
Valid driver’s license and auto insurance
Minimum 2 years of RN experience
Strong clinical judgment, communication skills, and the ability to work independently

Nice To Haves

Hospice experience preferred
Experience with HomeCare HomeBase is a plus

Responsibilities

Perform comprehensive initial and ongoing patient assessments, addressing physical, functional, psychosocial, and environmental needs
Develop, implement, and adjust individualized care plans to ensure continuity and quality of care
Determine hospice eligibility and level of care through admission and recertification criteria
Educate patients and families on disease progression, symptom management, and end-of-life care
Coordinate care with physicians, social workers, chaplains, and the interdisciplinary team
Provide clinical direction and oversight to Hospice Aides and LPNs/LVNs
Document care thoroughly to reflect patient progress and outcomes
Respond to changing patient needs and communicate updates to the care team
